Health History – Now Paperless

If you've been to our office lately, you may have noticed a change with our check-in process. We have introduced electronic health history forms. Now, when you check in, you have an opportunity to review your demographic and medical health history information via an iPad.

This is just one example of our commitment to sustainability and how we aim to be a green dental practice – no more paper forms! In fact, for new patients we have now reduced paper printing to only one sheet – from when you call to schedule, until you check out and insurance is processed!

With these new forms, you will find that your information better integrates with our software so that our team has the most current information available for your appointment. Looking at overall health is important to us as there can be links to what is going on in your mouth with the rest of your body. Having your current health information allows us to best treat or advise you on any dental conditions.

Recognizing this is a change for you and us, we ask that you please arrive for your appointment 5 minutes early to get familiar with the forms on the iPad and update your information. Certainly, if you need any assistance, we are here to help. As we go through this transition process, please don't hesitate to share any feedback, or ideas how we can make the experience better for you.
